Follow steps 1 - 3 below to
begin your real estate loan application!
1.
APPLICATION. This
service is provided for customers and/or
properties located within our local lending area, including Black River
Falls, Jackson County, Wisconsin and surrounding communities.
Choose the loan type below and print the application.
You will need Adobe Reader
to view and print forms. Get a free copy at http://www.adobe.com.
A signed Purchase Agreement. (This
is not necessary for a pre-approval or a refinance.)
A completed application. Be sure
to include: List of current loans and credit lines with account numbers and
addresses. Address and phone numbers for all employers within the last two years.
The last two years' W-2s. We will
also need two years of tax returns (including all schedules) if you
are self-employed.
Original pay stubs for the last 30 days
(consecutive dates). We cannot use handwritten pay stubs.
Two months deposit statements to cover
down payment, closing costs and reserves. (This would include
statements for checking, savings, mutual funds, retirement accounts
and other investments.)
If applicable: Divorce Decree,
purchase agreement on the home you are selling, YTD Profit and Loss
Statement and Balance Sheet if you own a business.
If loan request is for a refinance
of your present home loan, please include a copy of title
evidence (Title Insurance Policy).
If loan request is for a refinance
of your present home loan, please include a copy of your
homeowner's insurance declaration page.
We need all
borrower's signatures on the application.
Bring your
checkbook, so you can pay for a credit report or appraisal when you
apply.
3. SUBMIT For confidentiality and personal service, we ask that you bring in, mail
or fax your completed loan application and items from the checklist to
any of our our six locations.

your name, address, date of birth, and other information that will allow
us to identify you. We may also ask to see your driver's license or
